The airport first opened in 1937 as a private airport, but during World War II, it was taken over by the United States Army Air Corps and became known as Clarksville Army Airfield. It served as a sub-base for the larger Campbell Army Airfield in Fort Campbell, Kentucky, and was primarily used for basic flying training. After the war, it was transferred to the USAF Tactical Air Command and remained under USAF control until 1959, when it was turned over to the United States Army. The airport returned to public airport status in 1960.

Several airlines have provided commercial air service to Clarksville over the years, including Ozark Airlines, Southern Airways, Air Kentucky, Express Airlines II, and Commuter airline Prime Air. The only airline serving Barkley Regional Airport is Contour Airlines, which operates Embraer 145 flights to Charlotte Douglas International Airport in Charlotte, North Carolina. The airport was built in 1928 on 260 acres of land along U.S. Highway 41 and was funded by a city bond issue. On June 16, 1930, the Evansville Municipal Airport was dedicated, with runways measuring 100 × 1,200 feet and four inches thick, costing $30,000. The airport has since expanded multiple times, with a new terminal opening in 1950, and the original terminal being replaced in 1988 with the new William H. Dress Terminal, designed by Hafer Associates, with 140,000 square feet and ten gates.
